MDMM 2008 Registration

If you plan to attend MDMM 2008, fill out this registration form. The early bird registration and payment deadline is March 15^th. We suggest, however, that you register as soon as possible since later registration fees will be higher (see the page about payments for details) . A few notes about this registration form:

Your email address will be used for identification and notification, so please double check whether you correctly entered your current email address.
Fields in bold are required to register. You can update other fields later.
If there are non-english letters in your registration data (for instance Polish diacritics), make sure that your browser submits the form using the UTF-8 encoding.
If you fill out the registration form for a second time, all mandatory fields must still be filled in and the new data will overwrite your previous registration. To update only a few fields (for example, the contribution title or accomodation fields), please choose the "this is an update" checkbox. After the registration deadline, only corrections to previous registrations will be allowed.

You will be able to update or change this information later if needed using the same registration form until the final deadline for abstract submission (May 15^th). Students using the reduced conference fee option should declare their status in the field below.

Arrival & departure By letting us know when you are planning to arrive and depart, we can help you get to the conference venue in case of problems. We also plan to charter a bus (free of charge for participants) during the afternoon of June 23^rd from Wrocław Główny (central railroad station in Wrocław) to Hotel LAS in Piechowice, stopping at Wrocław Airport along the way. Another chartered bus will return the same way in the afternoon of June 28^th.

Accomodation & meals We will make room and meal reservations at MDMM2008 discount rates for you if you choose to stay at Hotel LAS (seen on the right), where the conference will take place. Due to possible limited room space (single rooms in particular), the organizers reserve the right change reservations - speakers and participants with presentations accepted by the MDMM 2008 organizing committee will have the highest priority.

However, keep in mind that payments must be made in cash or with a credit card. For more information and detailed instructions, please refer to the page about payments.

A less expensive alternative for accomodation is possible in Hotel TOPAZ (Szklarska Poręba, ul. Prusa 3), which is about 15 minutes by bus from the conference venue. If you plan to stay somewhere else than these two hotels, please let us know by selecting 'other' from the list.
